Physical Activity Readiness Questionnaire

This PARĀ­Q is designed to help you help yourself. Many health benefits are associated with regular exercise, and the completion of the PARĀ­Q is a sensible first step to take if you are planning to increase the amount of physical activity in your life.

For most people physical activity should not pose a problem or hazard. The PARĀ­Q has been designed to identify the small number of people for whom physical activity might be inappropriate or those who should take medical advice concerning the type of activity most suitable for them. Common sense is your best guide for answering these few questions. Please read them carefully and choose the YES/NO opposite the question as it applies to you.

1. Has your doctor ever said that you have a heart condition
and recommended only medically supervised activity?*

2. Do you have chest pains brought on by physical activity?*

3. Have you developed chest pains in the last month?*

4. Do you tend to lose consciousness or fall over as a result of dizziness?*

5. Do you have a bone or joint problem that could be aggravated by the proposed physical activity?*

6. Has a doctor ever recommended medication for your blood pressure or heart condition?*

If you have answered NO to all questions accurately, you have reasonable assurance of your present suitability for:

A GRADUATED EXERCISE PROGRAMME

A gradual increase in regular exercise promotes fitness and improves overall health while minimising or eliminating discomfort. Postpone entry into the programme if you feel unwell, or have a temporary illness.

I, the undersigned, agree that:

All scheduled training appointments will start and finish on time. I also understand that Roger Frampton may not be able to provide the full appointment time if I am late to an appointment. However, if my coach is late for an appointment, I will still receive the full appointment time or, where this is not possible, have the outstanding time added to subsequent appointment.

My answers to the questions posed in my consultation and PARQ form are true and complete to the best of my knowledge. I understand and agree that it is my responsibility to inform Roger Frampton of any conditions or changes pertaining to my health, now and ongoing, which might affect my ability to train safely and with minimal risk of injury.

The result of any training programme cannot be guaranteed and my progress depends on my effort, determination and cooperation, in and outside of my scheduled appointments. Individual results may vary and Roger Frampton does not guarantee any particular result.


Liability Waiver

I, the undersigned, being aware of my own health and physical condition, and having knowledge that my participation in any exercise programme may be injurious to my health, am voluntarily participating in physical activity directed by Roger Frampton.

Having such knowledge, I hereby release Roger Frampton, his representatives, agents, and successors from liability for accidental injury or illness that I may incur as a result of participating in the said physical activity. I hereby assume all risks connected therewith and consent to participate in said physical activity. I agree to disclose any physical limitations, disabilities, ailments, or impairments that may affect my ability to participate in said fitness program.
